TERMS OF TRADE

The following Terms of Trade apply to services provided by Kaiapoi Family Doctors. By signing this document, you agree to the terms and conditions outlined below:

15 mins Consultation:  Standard consultations are 15 minutes long  for one complex issue and two simple concerns. The doctor will advise you if multiple requests cannot be met within a standard 15-minute consultation, and deal with the most clinically urgent item at your appointment.

30 mins Consultations: For more than two simple concerns and or one complex issue, patients must book a double appointment (There is a double charge for a double appointment).

Payment: Kaiapoi Family Doctors request payment on the day of consultation unless prior arrangements are made.

·        If cost is a barrier to accessing care, please speak with the Practice Manager to discuss payment options.

·        Payment is accepted by cash, EFTPOS, credit card, or automatic payment.

·        If payment on the day is not possible, it must be made within 7 days of the invoice or as agreed with the Practice Manager.

·        We may refuse to provide any routine service if failure to meet agreed payment terms and a referral to a debt collection agency may be made. All associated recovery costs will be charged to the patient. This may affect your credit rating.

Appointments:

·        Patients accept that if no appointments are available or if clinically appropriate, they may be directed   to an after-hours or emergency provider.

·        If a patient arrives late for an appointment, they may be asked to reschedule.

·        At times, due to unforeseen circumstances, appointments may need to be rescheduled, sometimes at short notice.

Missed Appointments:   

1st and 2nd DNA - $25 DNA fee will be charged for 15mins appointment, $50 for a double appointment.

3rd DNA – Enrolment reviewed and may be terminated.

Late Cancelation / DNA: If you cancel within 2 hours of an appointment or do not show up, a minimum of $25 late cancellation fee will be charged.

Code of Conduct for Patients:

By enrolling with us, Kaiapoi Family Doctors expects all patients and accompanying persons to adhere to our Code of Conduct to maintain a safe and respectful environment for staff and patients.

The following behaviours are considered unacceptable and will not be tolerated:

·        Repeated failure to attend appointments without notifying the practice

·        Failure to pay for services without prior arrangement

·        Any form of aggressive behaviour:

Swearing or using abusive language

Making harassing, offensive, or intimidating statements

Physical assault or inflicting bodily harm

Intentional damage to equipment or property

Non-compliance with this Code of Conduct will result in immediate removal from the facility and/or

termination of enrolment. Adults are expected to supervise children in their care.
